When do you need the site to launch?
What do you need the site to do?
What are your needs for mobile or tablet devices?

What is your budget?
Who is likely to be leading the project?
Who is likely to be the decision maker on the project?

Do you already have a website?
What is the URL?
What do you like or dislike about it?

Here are some questions I ask when beginning the design process:

Describe your primary audience. 
Describe your secondary audience.

What is your desired message?
What feeling do you hope the design of the site to convey? 

What do you expect to be visitors' initial perception of your organization when they visit your site?
What is the desired perception you wish visitors to depart with?
